Service Provider collects various types of data from you and other authorized users, including basic information such as your name, address, e-mail address, etc., which is gathered from you as you complete the information required by the Eceptionist™ scheduling application. As part of the Eceptionist scheduling application, we will also collect personally identifiable information about the individuals for whom appointments are scheduled.
